Pupil Premium Funding Claim Request Form

Please complete this form to request a reimbursement for an item you wish to claim for. Please note that this will only be processed if it meets the criteria detailed on the previous page.

Upon receipt of this form we will process your claim if it meets the criteria as stated in the 'Allocations for Parents' summary document, and if you have sufficient funds remaining. Please note that claims must be received by 16/05/2026 to be processed during the 2025/2026 academic year. Claims received after this date will not be processed.
